Process  Step 1: Pull files of students who came from the Idaho Infant Toddler Program between July 1, 2007 & June 30, 2008 Child’s birth date must be between July 1, 2004 and June 30, 2005 Child’s birth date must be between July 1, 2004 and June 30, 2005  Step 2: Eligibility Report or IEP/IFSP adoption date on or prior to the child’s birth date?  Step 3: Check to see if services began as written on the IEP.  Step 4: If late, how many days late?  Step 5: Why was it late?

Scenario 1  5 children whose birthdays fell between July 1, 2004 and June 30, 2005 were referred from Part C Infant Toddler Program  3 of the 5 were found eligible for school services  2 were NOT eligible  Of those found NOT eligible, 1 eligibility decision was delayed because the parent had to travel with his job and was unavailable until 2 days after the child’s 3 rd birthday.

Record the Data 2 NOT eligible – 1 on time Auto populates with 1 late and 3 eligible 5 referred 1 transition was 2 days late: put a 2 in both boxes Reason: Dad on the road

Accurate Data  Child did not reside in the district on their third birthday.  Child received services from ITP from another state.  Districts did not give themselves credit as having a plan in place when eligibility had been determined and the team agreed to adopt the IFSP.  More answers in Q&A on opening page of this data collection site
