5 Indicator C8 Effective Transition Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by their third birthday, including: a. Transition Planning; b. Notification to school district; and c. Transition conference 5

8 Step 1: Transition Planning at Initial IFSP Service Coordinator will….  discuss transition planning at the initial IFSP  record date and discussion in the IFSP and Babytrac  continue and document discussions related to transition at each IFSP 8

9 Step 2: Transition Notification Reports containing directory information are sent monthly: 24 Month Report Over 24 Month Report 30 Month Report 9

10 Step 3: Transition Referral Each Service Coordinator is responsible for sending directory information using transition referral form. Referral must occur between age 2 (24 months) and 2.5 (30 months) If parents consent, additional info can be shared. If parents do not consent, directory information is still sent to school district. 10

11 Step 3: Transition Referral (continued) Referrals received for children age 29 months and older should be immediately referred to the school district and noted as “late referral” on the transition referral form. Date of referral is recorded in Babytrac database. 11

13 Step 4: Transition ConferenceTransition Conference Scheduled no later than 2 years 9 months (30 months) Can be telephone or face to face Parent may or may not be interested in Part B services Must be documented on transition conference form and in the Babytrac 13

15 Notification Part C to B 24 Month Notification & Clients Over 24 Months with IFSP During Month By School District  Part B Responsibilities  Do not enter in Excent (Not a Referral)  Verify LEA residency  Planning Tool for Conferences, Evaluations, & Programs

16 Referral  Referral to Part B” for inclusion in B-12a occurs when LEA has been notified that child served in Part C is potentially eligible for services under Part B  Indicated by the Receipt of: SDE Clients 30 Months by School District -Individual Referral FormIndividual Referral Form

18 18 Late Referral to Part C – Part B: Part B Responsibilities  Defined as: Children referred to Part C ≤ 90 days from child’s 3 rd birthday and “served” by Part C,  Noted on BabyNet referral by check box for “LATE TRANSITION ALERT”LATE TRANSITION ALERT”  Reports in B-11 and in B-12e (OEC subtracts B-12e from B12-a)
