RIVER DELL HIGH SCHOOL HEALTH OFFICE INFORMATION Kathryn Kreger RN, CSN

Requirements for Enrollment Required for ALL INCOMING STUDENTS OR students born AFTER January : Meningococcal containing vaccine (Menactra) Tdap (Tetanus, diptheria, pertussis) Physical examination within 365 days upon entering district Mantoux screening for students entering from selected countries.

Physicals Two part form Part A- Health History Questionnaire Part B- Medical Athletic Examination Form Available on website, must be accompanied by Athletic Permission Slip and Health History Update Form if physical is older than 60 days. The physical should be returned to the Athletic Office if student will be participating in a sport. Entrance physicals to be submitted to the Health Office.

Medications Bring all medications to nurse in original labeled container. Medication Administration Form filled out by BOTH the parent and the Physician. No Medication will be dispensed without proper documentation. BY N.J. LAW- School nurses are not permitted to give OTC meds w/o MD script.

Medications cont’d Health office DOES NOT supply over the counter meds. Asthma Action Plans and Allergy Action Plans are required along with 1 wallet size photos per medication. Reminder: Medication orders are good ONLY for the school year and must be renewed annually.

Illness in School A child sent home with fever must be fever free without fever reducing medication for 24 hrs before returning to school. Must be without vomiting OR diarrhea 24 hrs. before returning to school Suspected Strep or Conjunctivitis requires a clearance note from MD to return to school.

Injuries Students arriving to school with an injury, crutches, or cast must see the nurse and have an MD note. After treatment a Cleared to Return to School/Gym/Sports note is required. Appliances without a doctor’s note will not be allowed in school.

Gym Excuses All requests for gym excuses MUST come to the nurse. ONLY the Nurse can grant excuses. An excuse for more than 3 days requires a note from the doctor and must be presented to the nurse.

Absences and Tardiness Please call the Attendance Office at (201) to notify school of an absence OR tardy situation.

Medications at Home and Other Medical Conditions Please provide the Health Office with names, dosages, frequencies of medications taken at home as well as updates and changes. Please provide the office with information about any underlying medical conditions even if you think it may NOT be an issue at school. This information provides the nurse to address your child’s needs at school.
