Health issues – allergies, vaccinations Please answer the 3 questions on the form. If any of the answers are ‘Yes,’ please make sure you complete the rest of the question – giving as much detail as possible. Other factors– as above; please give as much info as possible. Rather have too much than too little!!

7 All INFORMATION disclosed is in the STRICTEST CONFIDENCE and will only be shared with the relevant people, that need to know. Anything from bed wetting to possible home sickness is really good to know. All issues are dealt with the utmost sensitivity. Please speak to any of us with ANY of your concerns.

8 Medication Permission Form
Please ensure you answer the VERY FIRST question on this form - Will your child require any medication (prescribed or over the counter) on this trip? YES/NO (delete as appropriate) If the answer is ‘Yes,’ please fill out the form, with as much detail as possible. E.G. Name of medication (in full) Dosage needed Frequency needed and when (e.g. every 2/ 4/ 6/ 8 hours and also before/ after or with meals)

11 Labelling Medication Please label all medication that is sent in for your child, including the blister packs!

12 We will not give any medication to your child, that has not been sent in by you.
We have first aid supplies. By law, these do not include any medicines. We encourage you to send in paracetamol and sickness tablets – Just in case! *we return all un-used medication.

13 Inhalers Please ensure that the inhalers are in date and that there’s enough to last the whole trip. In total, we ideally need 2 blue inhalers; in case one goes missing! One of these needs to be sent into school prior to the residential. The second inhaler needs to be bought into school on the morning of the residential, and shown to a member of staff at one of the First Aid stations. This inhaler will be the responsibility of your child – they will need to keep it with them at all times – day and night. *please note that if your child has a preventer inhaler this also needs to come in; but again it will be their responsibility.

14 Epipens Please ensure that the 2 Epipens are in date!
In total, we need 2 Epipens– One of these needs to be sent into school prior to the residential – to be put in the first aid box. The second Epipen needs to be bought into school on the morning of the residential, and given to a member of staff at one of the First Aid stations – this will then be passed on to your child’s group leader (member of staff). It will then be kept with your child’s group leader at all times.
