2. Immunizations Tdap (Tetanus, diphtheria, acellular pertussis)
One dose of the Tdap vaccine to be given no earlier than the 10th birthday
Meningococcal conjugate vaccine
One dose of the Meningococcal conjugate vaccine applies to students when they turn 11 years of age and attending 6th grade

3. AGE REQUIREMENT
If your child will not meet the age requirement to receive the state mandated immunizations prior to the first day of school, please provide a physician note indicating the date your child has an appointment to receive these immunizations.
Failure to provide this documentation will result in exclusion from school on the first day.

4. Religious Exemption
If your child will not be receiving the required 6th grade immunizations because of a religious exemption, please provide the health office with a letter stating this belief. This letter must be provided to the health office prior to the first day of school to prevent exclusion.
If your child has never received any childhood immunizations because of your religious beliefs and you have already provided a letter stating your beliefs, no additional documentation is needed.

5. Medical Examination
Best practice states that children should have at least one medical examination for each developmental stage. Students attending Medford Lakes School District will be required to provide a physical for entry into school either Pre-K/Kindergarten & for 6th grade. All medical exam forms/updated immunization records are due by 8/24/ Please notify the health office if your child’s annual medical appointment is scheduled after this date by sending an to Your child will be excluded from school if the physical form is not returned to school within 30 days.

6. Camp Ockanickon
Confidential Health Information Form & all physician forms for medication administration
Please return these forms to the health office 10 days prior to the date of the trip.
This form will ask for the parent to indicate information such as fears, difficulty either falling or staying asleep, sleep walking occurrence, bed wetting, and if your child has ever slept overnight away from home.

7. Consent for the Administration of Approved Medication
The OTC medication form must be completed and returned prior to camp for your child to receive any medication that is indicated as part of the Standing Orders signed by Dr. Burger our school physician. Examples: Tylenol, Motrin, TUMS or Benadryl.
You may not send your child to camp with OTC medication in his/her duffel bag. Students will not be permitted to self–administer any medication while attending camp.

8. Medication Administration
All other medications that may need to be administered to your child other than Motrin, Tylenol, TUMS & Benadryl will require a physician note. Forms will be provided to you in your child’s camp packet that should be used for this purpose.
Examples: Claritin, Zyrtec, Flonase, ADHD medication not usually taken during the school day, antibiotics, and any vitamins/supplements

9. Camp Tips
All medication must be supplied to the nurse in the original prescription bottle.
Check all expiration dates
Medication must be dropped off to the health office with the completed paperwork 10 days prior to camp. Please plan on spending at least 10 minutes in the health office when dropping off medications.
The main office will not be able to take medication for camp.
Noncompliance will result in the parent being responsible to go to camp to administer medication to his/her child.

10. Asthma Action Plan
In order for your child to either self-administer an inhaler or receive medication administered by a nebulizer, the nurse must have a current AAP for the school year.

11. Anaphylaxis Emergency Plan
Any student who has a life-threatening food allergy must have a current Anaphylaxis Emergency Care Plan in place.
Parent must provide emergency medication
The school nurse will forward parents who have students with life-threatening food allergies information regarding the menu at camp and contact information for the chef.
Please contact the nurse for any other dietary concerns such as a gluten-free diet.

12. Physical Restrictions
If your child has a physical restriction such as no PE/Recess, due to an injury or illness, he/she will not be able to participate in the physical activities at camp.
