Out-of-State Monitoring

Target Points Blanket Release statement between DHHR/WVDE; grants access to necessary student assessments/records. Creating a referral process within West Virginia and with Out-of-State placements to notify the Out-of-State Transition Specialist when a student is being ordered Out-of-State for treatment and when the student is returning. Developing the transition process for Out-of-State returns, through collaboration with Regional OIEP Transition Specialists, LEAs, Counselors, and DHHR Staff.

Capturing the Whole Student and Implications 265 Students are currently placed in PRTF and Group Residential Care (This totals includes students not verified WVDE, but tracked by DHHR). On average, students reside in placement for 287 days.
