Upon logging in and selecting the Service and Transition Planning link, providers will be taken to the “Referral Tracking” pages where they can view non-identifying information about children who have been authorized to receive services. DSS and acute care users will also be able to view the “Referral Tracking” pages.

3 Understanding the Referral Tracking List data elements
Transition Planning Understanding the Referral Tracking List data elements The following consumer data elements, mainly from FamilyNet, are viewable on the Referral Tracking List: Referral ID – Internal service referral number assigned by FamilyNet. Referral Category, Program, and Model – Indicates the type of service, such as residential – residential treatment – chapter 766 DSS is currently targeting for the child Gender Age – Calculated age of the child requiring services in years and months Current Setting – Name of the facility at which the child currently resides AND Date – Date on which child was placed on Administratively Necessary Days (AND) status Length of Stay – Number of days child has been at current facility Region and Area – of associated DSS case If you would like addition information regarding a specific child: Click the “View Details” link to view the “Referral Profile” report. Contact the Area Office or Lead Agency to discuss specifics regarding this child.
