Second Screen (not shown) –Member Number : Enter your social security number –Zip Code: Enter your home address zip code –Click Submit Third Screen (not shown) –Follow instructions on the screen –User Name: Social security number – or change to what you want –Password: Follow instructions on the screen for setting your password –Confirm Password: Type password again –Security Question: Choose the question you want –Security Answer: Type your answer – case sensitive – Address: Must give a valid address –E-notify: Check mark the box if you want an sent to you when we receive a claim Please put your password in a location that you will be able to find it.
