June 9, 2015Ministry of Municipal Affairs and Housing 2 Purpose Describe the value of Local Health Integration Network (LHIN) – Service Manager collaboration: Improving outcomes for Ontarians and achieving greater value for money invested Leveraging the service system management roles of LHINs and Service Managers to meet the unique needs of communities across the province Provide an update on collaborative work underway to identify opportunities to strengthen coordination between the housing and health sectors Discuss next steps, including the recently launched Long-Term Affordable Housing Strategy (LTAHS) Update consultation process

June 9, 2015Ministry of Municipal Affairs and Housing 3 Improving Outcomes for Ontarians The housing and health systems provide intersecting services for many mutual clients, including frail seniors, persons with physical disabilities, and persons with mental illness and/or addictions Improving outcomes for people with complex needs is a priority for both systems Coordinated service delivery helps people to manage and/or recover in the community, reducing the burden on emergency services (such as repeat emergency room visits or emergency shelter use) There are a number of existing collaborative initiatives already underway between LHINs and Service Managers, both at the strategic planning and project levels

June 9, 2015Ministry of Municipal Affairs and Housing 6 Examples of Successful Collaboration…. just to name a few (2 of 2) City of Greater Sudbury 32 seniors housing units with dedicated space in building to provide Community Hub support services to residences and surrounding community members (Cedarbook – Walden Seniors) Region of Waterloo 32 seniors units including rent-geared-to-income units, 4 wheelchair accessible units, annual operating funding for care and support services for seniors - adjacent to long-term care facility (Sunnyside Seniors Apartments) Haldimand County 21 unit seniors project with annual operating funding for support services (Halton Cheshire Homes) County of Simcoe (Penetanguishene) 40 unit apartment complex situated within housing community consisting of 143 bed long-term care facility, 57 life lease housing options, including 17 semi-detached manor homes and 40 apartment suites, 42 assisted living apartments (Georgian Village)

June 9, 2015Ministry of Municipal Affairs and Housing 7 Engagement to Strengthen Collaboration MMAH and MOHLTC are working together with our partners to identify opportunities to strengthen coordination between the housing and health sectors Recent engagement activities have included: LHIN-Service Manager Collaboration Advisory Group: Representatives of LHINs (Central LHIN and North East LHIN CEOs) and Service Managers (Toronto, Durham, York, and Cochrane DSSAB senior housing staff) were engaged in November 2013 and February 2014 Discussions at Existing Tables: Including the Service Manager Housing Network, LHIN CEOs, Northern Ontario Service Deliverers Association, Urban Social Services Commissioners, and Mental Health and Addictions Leadership Advisory Council December 2014 Stakeholder Engagement Session: Representatives of LHINs, Service Managers, Community Mental Health Agencies, Housing Providers, and People with Lived Experience

June 9, 2015Ministry of Municipal Affairs and Housing 8 On December 3, 2014, MMAH and MOHLTC held a one-day focus group to seek input on the challenges and opportunities of the mental health and addictions supportive housing system and LHIN-Service Manager collaboration The feedback received was categorized in four overarching themes: 1) Clarity around role and responsibilities 2) Alignment of accountability, planning and/or funding mechanisms 3) Improving access to housing and/or support services 4) Updating programs to reflect best practices Feedback from December 3, 2014 Stakeholder Engagement Session

June 9, 2015Ministry of Municipal Affairs and Housing 9 On October 31, 2014, MMAH and MOHLTC ADMs sent a joint memo to LHINs, Service Managers and Aboriginal Program Administrators (APAs) which: Advised LHINs and Service Managers / APAs of the opportunities provided by new investments under the Investment in Affordable Housing program extension and Mental Health and Addictions Strategy Phase Two to strengthen collaboration Encouraged LHINs and Service Managers / APAs to meet together and discuss local mental health and addictions needs, and to identify opportunities to work together to address these needs Current Opportunity

June 9, 2015Ministry of Municipal Affairs and Housing 11 MMAH and MOHLTC are continuing to work together on how we can strengthen alignment at the provincial level, remove barriers and support local collaboration. Long-Term Affordable Housing Strategy (LTAHS) Update consultations were launched on Tuesday, April 14 (TBC): Information on the consultation process including the Discussion Guide can be found at The consultations are seeking input under four themes: 1.A Sustainable Supply of Affordable Housing 2.A Fair System of Housing Assistance 3.Coordinated, Accessible Support Services 4.Evidence and Best Practices The process to develop the LTAHS Update related to coordinated, accessible support services will include work with partner ministries, including MOHLTC, MCSS and MCYS, and reaching out to a wide range of sectors through existing tables for their input.
