June 9, 2015Ministry of Municipal Affairs and Housing 3 Improving Outcomes for Ontarians The housing and health systems provide intersecting services for many mutual clients, including frail seniors, persons with physical disabilities, and persons with mental illness and/or addictions Improving outcomes for people with complex needs is a priority for both systems Coordinated service delivery helps people to manage and/or recover in the community, reducing the burden on emergency services (such as repeat emergency room visits or emergency shelter use) There are a number of existing collaborative initiatives already underway between LHINs and Service Managers, both at the strategic planning and project levels

June 9, 2015Ministry of Municipal Affairs and Housing 11 MMAH and MOHLTC are continuing to work together on how we can strengthen alignment at the provincial level, remove barriers and support local collaboration. Long-Term Affordable Housing Strategy (LTAHS) Update consultations were launched on Tuesday, April 14 (TBC): Information on the consultation process including the Discussion Guide can be found at The consultations are seeking input under four themes: 1.A Sustainable Supply of Affordable Housing 2.A Fair System of Housing Assistance 3.Coordinated, Accessible Support Services 4.Evidence and Best Practices The process to develop the LTAHS Update related to coordinated, accessible support services will include work with partner ministries, including MOHLTC, MCSS and MCYS, and reaching out to a wide range of sectors through existing tables for their input.
