FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Overview Throughout the history of Friendship Village Chesterfield, the organization has made a commitment to “give back” to the communities it serves. Located in Chesterfield, Missouri, the organization is home to more than 400 seniors and employs more than 300 people from the St. Louis metropolitan region. The following report shares the Social Accountability results for Friendship Village Chesterfield for Fiscal Year 2013 (July 1, 2012 through June 30, 2013) and explains in detail the organization’s contributions.

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Overview Purpose & Responsibility According to the organization’s Articles of Incorporation, Friendship Village West County, doing business as Friendship Village Chesterfield, was organized on September 9, 1973 for the purpose of constructing, building, owning, operating and maintaining a home for the senior community. Its purpose is to provide housing, nursing care, and other services. Our facilities to enable its elderly residents to live safe, productive and independent lives. We also satisfy their needs for housing, health care and financial security. Friendship Village Chesterfield offers a full continuum of services for residents, including independent and assisted living, as well as skilled nursing services. A local board of directors governs Friendship Village Chesterfield and contracts with LifeCare® Services of Des Moines, Iowa for management services. Friendship Village Chesterfield, and its sister community Friendship Village Sunset Hills, are the only two senior living communities in the St. Louis region to provide LifeCare® services to residents. A not-for-profit 501(c)3 organization, Friendship Village operates under the guidance of the following mission and vision statements.

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Overview Mission Statement: The Village shall be operated by high moral standards according to Biblical guidelines; in conducting its person and business affairs, in making the Village each resident’s home, in caring for each resident and staff member, and in providing opportunities and challenges for the growth of each resident and staff member. Vision: To develop and maintain an environment to serve senior adults, with a caring and self-sustaining community that will provide facilities, security and health care with excellence as their hallmark according to Christian principles.

We are a faith-inspired, mission-driven organization that exists for those we serve. Within this report you will find how we are able to serve the St. Louis Seniors of today and of the future. Throughout the history of Friendship Village Chesterfield, the organization has made a commitment to “give back” to the communities it serves. In the next few pages the measurement of our personal goals will be laid out for you. Our personal goals help us measure our success against the industry standards. At Friendship Village Chesterfield we are always striving to drive our performance for all of St. Louis to see. Friendship Village Chesterfield is focused on our residents physical, emotional, intellectual, social, and spiritual needs through their journey with us. Our staff takes pride in how we operate as a family and that family atmosphere carries over to the residents and families we serve. The report that follows will show the achievements of our residents and employees who make up the Friendship Village Chesterfield family. Our organization is home to more than 400 seniors and employs more than 300 people from the St. Louis Metropolitan region. We are proud of each and every day that we spend together. FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Resident Volunteerism Our Residents’ selfless acts of volunteering their time speak louder than words. In FY residents volunteered inside the Friendship Village Chesterfield Community, thus contributing 8,580 hours. Our residents not only volunteered within our walls but took it to the community and with 17 residents volunteering they provided 4,075 hours to the community at large.

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Employee Volunteerism The Employees of Friendship Village Chesterfield go above and beyond their work load. In FY2013, 10 of our employees volunteered in communities across the St. Louis metropolitan community and contributed 860 hours.

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Employee/Resident Charitable Activities Friendship Village Chesterfield made donations to local deserving charities and community organizations. With the help of our residents, board members, and employees the following organizations received our support. YMCA Donation: $ Donation of 10 walkers to HUD residents: $ Komen Race for the Cure: $ Angel Tree Benefiting children of incarcerated people. $2, Book Donation to Salvation Army: 100 books per month. $1, Donation to Chesterfield/Fenton and Chesterfield Chambers for golf outings. $1, STLPDA Group Fashion Show and Luncheon. $ Girl Scout/resident volunteerism. $ Chamber of Commerce Memberships: Chesterfield: $775.00, West County: $ Kirkwood/Des Peres: $ Crestwood/Chesterfield: $ Fenton: $1,000

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Community Support Resident Volunteerism with Claymont Elementary School: 35 people participated contributing 3464 hours and donated $ West County Day School/benefiting St. Jude’s. 32 Hours volunteered with $ donated Boy Scout Variety Club Concert $ donated and 320 hours volunteered. Intergenerational programming with River Bend Elementary Orchestra: 222 hours volunteered, $ donated.

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Educational Outreach Friendship Village Chesterfield hosts for meeting space and or provides lunch to 400+ plus Chesterfield professionals. Medicare Seminar: Provide education to assist residents in choosing a plan Benefit Seminar for Veterans and their spouses for Friendship Village residents and community at large Participate in job fairs for employment recruitment Transportation, space, and activities in partnership with YMCA of Chesterfield and Chesterfield’s LOAP program Friendship Village Chesterfield provides educational programs to the residents, employees and general public.

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 O UR C HAPLAIN : J AKE B ENNETT Funeral/Memorial Services: 24 services have been planned and conducted. Village Care Center Visits: Pastoral care averages 25 visits per week. Residents/Employees visits annually Outreach Ministries: Mentors seminary students by preparing them with end of life issues. Chaplain Bennett presents regularly presentations at area churches to raise awareness about end of life issues and personal Counseling to approximately 11 residents and employees each month. Ongoing Counseling: Covenant Theological Seminary: Seminary students provide free counseling for the residents. Chaplain Bennett has devoted eight hours of training with them, along with an additional two hours speaking to a group of seminary interns. Friendship Village Chesterfield has one Chaplain and one Assistant Chaplain to meet all the spiritual needs of our residents and staff. Our chaplains also visit our hospitalized residents. Average visits are 4-5 visits each week throughout the year.

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Resident Spiritual/Counseling Support Friendship Village hosts trips to Israel and Washington DC each year with the chaplain. Israel Trip: 6 residents attended Washington DC Trip: 8 residents attended Bible Studies: 59 residents, on average, are in attendance among the five weekly Bible Study classes. Three devotions are held each week attracting an average of 40 residents altogether. Worship Services: Friendship Village Chesterfield, offers two worship services each Sunday Approximately 90 residents attend worship services hosted in our activity room and Village Care Center

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Personal Property Taxes Real Estate: $345, Personal Property: $36, Total: $ 381,766.34

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Benevolence Friendship Village Chesterfield provides Benevolent Care if residents resources diminish as needs increase. We thank our generous donors that help our residents at this time of need of restoring their hope and protect their dignity. The need for benevolent care for residents at Friendship Village Chesterfield continues to increase. In 2013 over $50,000 was needed to help bridge the gap between individual residents’ financial means and the actual cost of care at Friendship Village Chesterfield.

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 Friendship Fund We are fortunate to stay true to our mission because of the generosity of others to our Friendship Fund. Friendship Village Chesterfield has not altered the level of care and service it provides, despite the fact that costs continue to increase and the government continues to offer fewer subsidies for long-term care. Friendship Village Chesterfield has contractual agreements with Medicare, Medicaid, other third party insurances. Regardless of the reimbursement received even when it falls short of the cost of care, Friendship Village Chesterfield will continue providing comprehensive care.

FRIENDSHIP VILLAGE CHESTERFIELD S OCIAL A CCOUNTABILITY 2013 In the Year Ahead Friendship Village Chesterfield will continue to work with our community and various organizations to enrich the lives of our Friendship Village family. Below are just a view things we have planned for next year.: Volunteer opportunities to help residents stay involved in the community Educational programs to support residents in staying active Benevolent Care program to enrich the lives of our residents Recycling possibilities to engage our Friendship Village family and community to living healthier Community Engagement occasions to connect residents with officials and legislators Interactive events to enhance employee involvement in their community
