NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road General Scope of Work Widening of NY Route 347 with additional travel lanes and shared-use path for pedestrians and bicyclists in a Manner that Maintains the Corridor Theme; Pedestrian facility design and installation; Bus turnouts and enhanced bus stop design and installation; Extensive landscape design and installation including, but not limited to: vegetated screening, planted raised median, street trees, vegetated drainage features; Invasive species removal as shown on the Indicative Plans Concrete sound wall design and installation; Pedestrian facility design and installation: Improved sidewalks, curb ramps, signals, and crosswalks for pedestrians. New signal at the Green belt trail crossing. There are four sound walls proposed in this section. We have community consensus from three community and waiting for one more.

Relocation and/or preservation of existing utilities; NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road General Scope of Work (Continued) Relocation and/or preservation of existing utilities; ITS Installation and integration with the existing INFORM system; Traffic signals; Installation of street and pedestrian lighting. Signing & pavement markings; Work Zone Traffic Control; Drainage; GreenLITES Certification at Evergreen Level; There are total three signals- two existing and one new. Includes installation of pedestrian level lightings at median crossings, bus stops and green stop. Relocation of existing street lights. Water quality basin is proposed on the north side east of the culvert crossing. The scope also includes culvert replacement.

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Proposed Typical Cross Section New Sound Wall(s) or Low Screen at/near ROW Line 3.3. m lanes with 1.8 m shoulder in general. The shoulder width will be 1.2 m where right turn lane exist. A 1.2m imprinted concrete shelf and 3.6m shared use path on south side and 1.5m sidewalk on north side. Imprinted raised concrete median with planting and potential sound wall at the ROW line.

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Aesthetic Design 13 Think holistically about how the design and construction decisions will affect the final aesthetic design of the roadway. Ensure the RFP requirements as it relates to the Aesthetic Design Guide are met throughout various stages of the project. The Technical Solutions should reflect a collective design of related elements: Tree Inventory, anticipated vegetation removals, and proposed planting Aesthetic design of concrete materials ie. sound walls, retaining walls, and median barrier Shared-use path alignment, Greenway Stop, and associated transportation features ie. bio-swales, drainage basins, rain garden, etc Bus stop enhancements ie. decorative pavement, bus shelter, solar lighting, bike racks

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Adjacent Projects   PIN/Description: 0054.20 (D900012) – NY 347 Safety and Mobility Improvements from NY 111 to Mt. Pleasant Road This is a Design-Build project with anticipated completion date of May 2015. WZTC: Long term lane closures Design Build team: Posillico Civil and AECOM

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Right-of-Way The Department anticipates that all additional ROW it deems necessary for the Project will be acquired prior to the Notice to Proceed (June, 2014) in accordance with DB §107-22. No relocations are required as a result of this project. Should the Design-Builder identify any additional ROW needs for the project, it shall be acquired in accordance with DB §107-22.

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Utility Coordination – Existing Utilities In accordance with Part 4 – Utility Requirements, Design- Builder is responsible for: Appointing a single dedicated person responsible for managing all utility coordination; Verifying utility facility locations; Identifying & resolving potential conflicts not yet documented; Providing information on the need for any additional utility easements; Coordinating with owner for utility relocation; Provide stakeout and final layout for utility relocations Protecting existing utilities; Cost to repair utility damage caused by Design-Builder; Informing the Department of all meetings/correspondence with utility owners; Being Party to and signing the Three-Party Utility Agreement.

Long Island Power Authority (LIPA) and Verizon (Aerial): NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Utility Coordination – Existing Utilities Long Island Power Authority (LIPA) and Verizon (Aerial): Approximately 62 poles carrying electric, telephone and cable on both sides of NY 347 (to remain in service); Poles are owned by LIPA or Verizon- LIPA owns 36; Verizon owns 26. The current pole heights vary from approximately 35 feet to 45 feet; Service connections to businesses and homeowners originate from the pole line along NY 347 (to remain in service); There are third party attachments, including Cablevision and LI Fiber; There are 4 overhead transfer lines that feed to the communities to the north and south of NY 347.

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Utility Coordination – Existing Utilities National Grid Gas There is an existing 8” steel main in the center median of NY 347; National Grid shall perform all work to relocate this main out of the center median; This relocation is anticipated to take 6 weeks; There are steel gas mains located on the east side of Terry Rd. – a 4” main south of NY 347, and a 6” main north of NY 347. If offsets are required due to pavement widening and/or drainage modifications, National Grid anticipates requiring approximately 2 weeks to complete this work once the site has been marked out and made available.

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Utility Coordination – Existing Utilities Verizon (Underground): There is a seven (7) conduit - 4” PVC duct bank with a buried copper cable along the existing south side of NY 347 from Mt Pleasant Road to Hidden Ponds Drive, with approximately 30” cover; There are 3 manholes associated with these conduits; these manholes are to be considered fixed objects which cannot be relocated; There is also an underground 15 conduit PVC duct bank (5 across and 3 deep) that runs along the east side of Terry Road that and crosses NY Route 347; Manholes associated with these conduits are located either side of NY Route 347; additional manholes exist within the project limits on Terry Road; Should complete relocation of duct bank and buried cable between manholes be required, Verizon will need at least 10 weeks to complete the work; Verizon shall be responsible for all relocation work; Verizon will require approximately 2 weeks to offset conduits and/or cable at each conflict location;

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Utility Coordination – Existing Utilities   Suffolk County Water There is a 12” water main on NY 347 in the vicinity of the center median from Mount Pleasant Road to Terry Road and beyond; These water mains shall be relocated to the south side of NY 347 by the Design-Builder; The Design Builder shall be responsible for all costs to design and reconstruct these mains as well as all service connections, hydrants and valves There is also a 12” water main that runs along the west side of Terry Road.

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Contract Information Lump Sum Design-Build Contract; Best Value Selection Process (50% Price, 50% Quality); Project Completion Date shall be the earlier of: April 30, 2016; or The number of days past NTP proposed by the successful Proposer on Form SCD (ITP Appendix D) There is one Interim Completion Milestone (Permanent Transfer of all Traffic to Final Lane Configuration) There is a Payment Schedule provided in the RFP; $ 80,000.00 Stipend to Unsuccessful Proposers; Liquidated Damages may be imposed for not meeting the Interim Completion Milestone date and the Project Completion date; Escrow Submission of Bid Documents for Successful Proposer;

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Contract Information There is a Buy America Clause There is an Iran Divestment Clause There is a DBE Participation Goal 15% All Environmental permits and ROW by the Department based on 30% design plans, including NYSDEC and USACE Fresh Water Wetlands Permits The Design-Builder is responsible for complying with the requirements of Environmental permits obtained by the Department, including submission of final detailed plans to permitting agencies for review and approval The Design-Builder shall be responsible for all costs and/or delays associated with inconsistencies between the final plans and the approved permits, including any necessary re-permitting and/or redesign. modification All Construction Related Permits (i.e., SPDES) by Design-Builder

NY Route 347- Design Build Phase 2 Mount Pleasant Road to Terry Road Quality Control (QC) QC is Design-Builder’s Responsibility Design-Builder shall appoint a QC Manager responsible for QC Plan for both Design and Construction Design QC shall be by the Designer in accordance with the QC Plan Construction Inspection shall be by an Independent Engineering Firm (Hired by the Design-Builder) Materials Testing shall be by an Independent Testing Firm or Laboratory (Hired by the Design-Builder) The Independent Testing Firm shall report to the Construction Inspection Firm Offsite Plant Quality Control will be performed by the Department

Proposals should not have links to external web sites. Proposals should contain all information needed for evaluation. Do not “assume the Department knows” your firm and it’s experience/qualifications. Make sure the schedule you propose includes all anticipated time associated with the final design, QC/QA review, production, and construction of project materials such as the concrete sound walls.
