NRC Findings on Arkansas Nuclear One Accident On March 31, 2013 a temporary lifting assembly collapsed while moving a 525 ton component (stator)of a turbine, in Unit 1 Stator fell through hole in floor onto a heavy transport vehicle One killed, eight injured 2

NRC Findings on Arkansas Nuclear One Accident Stator ruptured a water pipe in transport vehicle bay, causing flooding Certain electrical systems shared by Units 1 and 2 were damaged Emergency generators used for six days to supply power to cooling systems for Unit 1 3

NRC Findings on Arkansas Nuclear One Accident NRC concluded Entergy: – Approved a design for the temporary hoisting assembly that was not supported by detailed drawings, specifications, evaluations and/or certifications – Failed to review associated calculations to ensue that the assembly was designed to support the projected load 4

NRC Findings on Arkansas Nuclear One Accident – Failed to carry out a load test on the lifting assembly in all configurations for which it would be used – Did not ensure adequate supervisory and management oversight of the contractors and other supplemental personnel involved with the stator lift September, 2013 OSHA cited 26 safety violations and fines totaling $150,000 5

Agriculture and DOT to identify ways to improve operational coordination with State, local, tribal, and territorial partners; To enhance safety and security in chemical facilities; And to work with stakeholders to identify best practices to reduce safety and security risks in production and storage of potentially harmful chemicals Modernize polities, regulations and standards 8
