6 WILLFUL MISCONDUCT/RECORD FALSIFICATION AND NUCLEAR SAFETY CULTURE NRC Information Notice 2013-15 NRC discovered 7 recent willful misconduct incidents including record falsification An effective safety culture is essential at all phases of design, construction and operation; and can help prevent willful misconduct Ensuring expectations and consequences are clearly stated and understood.

7 WILLFUL MISCONDUCT/RECORD FALSIFICATION AND NUCLEAR SAFETY CULTURE A violation may be considered more significant than underlying noncompliance if it includes indications of willfulness. Key behaviors of a healthy safety culture need to be continually reinforces.
