Aged Care GP Panels Initiative4 Outcomes The outcomes to be achieved through the Initiative are: –Divisions to increase participation of GPs and Allied Health Service Providers in aged care homes’ quality initiatives; –Percentage of accredited Aged Care Facilities taking part in the Initiative to increase to at least 75% by 30 June 2008;

Aged Care GP Panels Initiative5 Outcomes –Residents of aged care homes have increased access to primary and medical care; and –Effective partnerships and collaboration established and maintained between GPs, allied health service providers, Divisions network members and aged care homes.
