Candidate Activity Form LEADERSHIP: Situations in which you had a leadership role – formal or informal A leader is someone whom others follow in direction, motivation or by example. For example – officer of a group, team captain, person who takes charge on an assigned project, SERVICE: Service is anything you do on behalf of others for which no compensation is given. Unpaid internships do not count as service since you are doing it on behalf of yourself and not others. Expectation is that service will be above the 15 hour school minimum. Please describe the activity in the table if you will not elaborate on it in the essay.

Candidate Activity Form Example: “My friends and I organized a non-profit, senior citizen prom in which residents of Meadowbrook could enjoy a formal, social event with their peers and high school students. We contacted local businesses for donations and handwrote 75 invitations and hand-delivered them to the residents.
