Public Services - Criteria Must meet at least one HUD CD National Objective: Benefit low- and moderate-income persons; Aid in the prevention or elimination of slums or blight; or Meet an urgent need. WP Consolidated Plan eligible and prioritized activities (supplemental handout) Area Benefit, Limited Clientele Activities, or presumed benefit LMI Benefit Objective – Minimum 51% low/mod – documentation required Must be a new service or an increase in an existing service

Application Process White Plains is NO LONGER using ZoomGrants. Applications and all necessary documents are available on the city website, http://www.cityofwhiteplains.com/index.aspx?nid=489, on the Community Development Funding for Public Service Programs section of the Community Development Public Services page. Anyone seeking assistance in the preparation of an application should contact the CD Office. Please refer to the Document Library on the same webpage as noted above for important information and resources to assist you with the successful completion of your application. Application completeness is critical and additional information may be requested. Applications will be accepted until 4:00 PM, Wednesday, January 31, 2018. Late applications will not be accepted.

Application Questions We will review them on the application itself and please keep the following in mind: All proposals are required to be consistent with federal and local plans for spending, as described in the City’s 2015-19 Consolidated Plan. (found on the City’s website for the 2015 – 2019 Consolidated Plan, at the City of White Plains Planning Department, White Plains Library, El Centro Hispano, and the Slater Center.) All beneficiaries/participants of proposed programs/services must be City of White Plains residents and verification records must be available for on-site monitoring. Funding is dependent on HUD’s issuance of the City’s annual CDBG agreement. CDBG sub-recipients will receive grant award letters upon the City’s receipt of HUD funding agreements.

Documents REQUIRED: List of owners, officers and board members, including home address, phone number and principal occupation Evidence of your nonprofit status Audit: A-133; Audited Financial Statement; or Annual Certified Financial Statement Articles of Incorporation W-9 Current Insurance Certificate Confirmation of Mandatory Public Hearing Participation (form available in the Document Library online) OPTIONAL: Letters from other organizations or agencies supporting your project Evidence of demand for your project (refer to Question #3 in Application Questions)
