Pseudocyst. (A) Axial noncontrast CT shows a nonspecific hypodense cystic lesion in the tail of the pancreas (white arrow) with a thin rim. Patient reported no history of pancreatitis. (B) Follow-up MRI of the abdomen postcontrast T1 weighted demonstrating a nonenhancing T1 hypointense lesion (white arrow). (C) Fat-saturated T2-weighted shows a simple appearing, centrally nonenhancing T2-hyperintense cyst (white arrow) in the tail of the pancreas with a thin peripherally enhancing rim. These findings remained nonspecific due to the absence of a history of pancreatitis. (D) EUS also showed a simple appearing anechoic round fluid collection in the tail of the pancreas. FNA yielded high concentration of amylase with no mucin consistent with a psuedocyst. The linear echogenic structure (arrow) represents the needle extending from the endoscope into the cyst.
