Reporting of accidents & near-misses at CERN For © cf. http://www.seahealth.dk 1 10 30 600 Serious or Major Injury Minor Injury Property Damage Accident Near-miss This triangle is coming form M. Bird’s studies. He was working for an Insurance company and in the 60s he analysed more than 1.5 millions accidents reported to the company and put in evidence that there is a direct relation between serious accidents and near misses with a ratio 1 on 600. Illustration with a lifting operation. In the picture we can see «lucky workers» as the load is falling just in front of them. Most of the time, this kind of accident will not be reported & no analyse will be conduct. But in the case where the workers would have been under the load... By analysing the event we will show that, for different consequences the causes are the same. Near-misses are warnings Actions must be taken at this level 16/03/2017

Analysis of accidents & near-misses at CERN The analysis of an accident is the responsibility of the DSO in the Departments or the LEXGLIMOS in large Experiments 3 type of accidents must be analysed as a minimum : - accidents resulting in falls - accidents that led to days of absence from work - accidents linked to the use of a vehicle on CERN roads The DSO or LEXGLIMOS defines if an analysis is required or not regarding : • the potential severity of the event (impact on the health, in the environment, in the installations and in the equipments), • the number of corrective and preventive actions which can be identified Evénements à analyser : principales causes d’absentéisme au CERN, i.e., accidents de chutes + accidents du travail avec arrêt + accidents liés à l’usage d’un véhicule motorisé ou non sur les sites du CERN Autres analyses à la demande de l’Unité HSE ou sur initiative des DSOs/LEXGLIMOS Other analyses can be requested by HSE Unit and/or are left to the initiative of the Departments 16/03/2017
