Think Local Act Personal – KARE New Directions Learning Event, Tullamore Court Hotel, May 18 th 2016.

Overall Change / Reconfiguration  Why we needed to change – New Directions National Policy  Who would we support in a different way: 9 individuals who use our service who live in two community houses in Naas to get their support from their home  How we delivered the support: Moved resources from day services to support individuals

Practical Steps taken Communication: Local level. Organisational level. Reconfiguring finances: Pay and Non Pay. Staffing

Challenges  Staff roles changing  Staff’s view that individuals would ‘miss local service or needed to start their day from local service’  Individuals missing the local service, seeing their friends, hearing the news!!  HIQA – change of focus for community houses to play an active role in the development of the Individuals person centred supported plan.

Overcoming the Challenges Staff benefit from changing shifts – work life balance Individuals supported to meeting their friends in different ways and at different times Individuals were sharing different / more varied news in the evenings Redefined our Individualised Planning Process with clear roles and responsibilities for all stakeholders.

Service Users opinion “I am a very busy man…I am meeting a lot more people than before. I love going to the Social Club in Sallins. There are so many women there that love me! I have a very busy week. I have great support from TLAP staff. I can now travel independently on the bus to Kildare for my course. I travel independently to Newbridge and my family home in Rathangan. You will never guess where I could be… You have to phone me if you need me. Leave a message and I’ll get back to you…."

Service Users opinion “It’s nice to stay in the house in the morning. I like to start the day in my own time and I love my morning routine. “I love having my own space in my apartment and staff support me to do the things in the community that I like to do; Aqua Aerobics, Bowling, Cinema, Swimming and my Voluntary Work”.
