‘Hearty Lives Craigavon’ MLB Theme 5 – Empowering Communities

What is Hearty Lives Craigavon? Nurse-led screening and prevention service Focused on CVD risk factors Targets Irish Traveller and BME groups in Craigavon area Partnership approach involves HSC, workplaces, GPs, Pharmacies, CV sector groups, local community health champions Funded by BHF,

Why? – evidence of need Clear evidence that Irish Travellers and BME communities, within legacy Craigavon Borough Council area, have increased risk factors for CVD than the rest of the population. All Ireland Traveller Health Study- ‘Our Geels’ (2010) Migration Observatory (2011) Putting a health inequalities focus on NI Cardiovascular Framework (PHA, 2011) NISRA-Migration statistics for NI (2010) SAAT Outcome monitoring report (2010/11)

How? – the day-to-day delivery Individual screening – BP, Blood Sugar, BMI Determination of risk Information/advice re: lifestyle Onward referral to GP, Pharmacist Facilitated access to community-based health improvement opportunities, e.g. stop smoking services, physical activity, nutrition, etc.

Outcomes Total beneficiaries = 6441 (including direct/indirect) 181 Irish Travellers have had heart health screening (Target = 140) 120 Irish Travellers have attended lifestyle change programmes (Target = 30) 18 Irish Travellers referred to GP 642 BME individuals have had heart health screening (Target = 600) 410 BME individuals attended lifestyle change programmes (Target = 180) 89 BME individuals referred to GP 22 individuals have been assisted to register with a GP 7 GP Practices have BHF information racks Heartstart delivered to 252

Increased awareness of risk factors for CVD among target populations; Increased opportunities for involvement in prevention of risk factors and uptake of healthy lifestyle programmes Increased GP registration Greater awareness of primary care and appropriate use of services Engagement of workplaces in staff health and wellbeing Tackling health inequalities

Legacy NINES (Northern Ireland New Entrants Service) incorporating learning from Hearty Lives Craigavon to ensure the specific health needs of BME groups are provided for in SHSCT locality.
