Driver of Vehicle Staff or Faculty Driver * A Purdue paid employee (Student Workers are not included in this category) Student & Volunteer Driver * Any driver that is not classified under the ‘Staff or Faculty’ category.

Vehicle Type University Fleet Van (4) 8 Passenger Vans are available for travel on IPFW business. Departments will be charged a daily rate of $15 per day, plus a mileage fee of $0.45 per mile driven. These vans are a better option for shorter trips. IPFW University Fleet Vans DO come with a gas credit card. Receipts must be kept and turned in with the van keys. Evans Toyota Rental Vehicle Rental vehicles are available for travel on IPFW business. Departments will be charged a daily rate based on the vehicle class, plus a refueling charge of $6.00 per gallon (if the vehicle is not returned with a full tank), along with a 4% excise tax. These vehicles are a better option for longer trips due to ‘Unlimited Mileage.’ Evans Toyota Rental Vehicles DO NOT come with a gas credit card. The driver is responsible for all gas charges incurred while renting the vehicle. Gas receipts should be given to the department for reimbursement on the Form 25.

Complete a “Self Assessment” by looking at the Self Assessing Chart on slide 8.Use of Vehicles for University Business Policy 3. Bring a valid Driver’s License when picking up the vehicle keys or verify we have a copy on file. 4. If there are multiple drivers listed on the Form 1, each driver must fill out a Driver's Release Form and complete steps 2-3. Send the signed forms to the Campus Safety Office in WU 127 or drivers can bring the forms with them when picking up the vehicle keys.Driver's Release Form All forms must be received by the Campus Safety Department before the start of the trip. Rates will be based on whole days only, not a portion. Picking Up the Vehicle: The driver must pick up the requested vehicle in person and sign a certification that they are in compliance with the Use of Vehicles for University Business Policy. If this is not possible please sign a Driver’s Release Form and send it with whomever will be picking up your vehicle keys for you. If you are departing prior to 10 a.m. in the morning, your vehicle will be available by 4 p.m. the day prior to your departure. The vehicle keys can be picked up at the Walb Information Desk between 4 p.m. and 6 p.m. the day prior to departing for no extra charge. Monday rentals can pick up the keys on Friday. Returning the Vehicle: Please only use the drop box if it is after office hours (Monday-Friday 7am – 6pm) or on the weekend. During Office Hours: Vehicle keys and all paperwork should be turned in to the front desk (Walb Union 127) After Office Hours: Please drop rental keys and all paperwork in the drop box. Record the date and time returned on the clipboard, which is located under the drop box. Also record ending mileage on the yellow slip in your key packet. Remove all personal items from the vehicle. Please return the vehicle in the same condition as it was picked up in. All rental vehicles should be returned to IPFW at the date and time that is listed on the Form 1. Additional charges may apply if the rental vehicle is not returned at the scheduled date and time, including next day early morning returns. NEXT
