Please be prepared to complete the online session to ensure that all your paperwork is completed accurately. Your time and effort to go through each of the slides will speed up the onboarding process. Time: minutes

Let’s begin with your paperwork…. Did you print a copy of the attached (NEW HIRE PACKET) in your welcome ? Please stop here and print out a copy to continue. Your paperwork needs to be completed neatly and mistake-Free. 

4 Paperwork attached…Use BlACK or BLUE INK ONLY
Drug-Free Workplace Please read it carefully. By signing this form, you are certifying that you are aware of working in a drug-free environment. Sign, print your name, and date it. User Technology Agreement By signing this, you are agreeing to abide by the User Technology Agreement set forth by MISD. Print your full name, title, campus, and then sign your name and date it. Workers’ Compensation Claim Reporting Notice By signing this form, you agree to the reporting notice set forth. Please sign your name, print your name, and date it. Statement Concerning Your Employment in a Job Not Covered by Social Security By signing this form, you understand the possible effects of the Windfall Elimination Provision and the Government Pension Offset Provision on potential future Social Security benefits. Don’t forget to print your name, and sign it. For employee ID, write your social security number. Demographic Data Form Complete all fields, including the shaded areas. An emergency contact name and phone number is crucial. Confidentiality of District Records Any and all information related to Finance, Student, and Human Resources must be kept confidential at all times. Please sign, print your name, date, and indicate department or campus and job title.

5 Paperwork continued… Employee Handbook Receipt
The MISD Employee Handbook (EH) is located on the district website at under Human Resources. By signing this form, you are accepting responsibility for reading it, and abiding by any changes. If you have any questions regarding the content of the EH, you are welcome to contact your immediate supervisor or Human Resources. You must print your name, campus, sign your name and date it. Employment Eligibility Verification (I-9) Read the instructions carefully. Complete Section 1 only, unless someone prepared or translated the information for you. See page 9 of 9 for a list of acceptable forms of identification. MISD must receive this information within 3 days of employment. If the acceptable documents are not received within 3 days, you may not begin employment. Only original documents are acceptable. Ex. A copy of your social security card will not be accepted. No mistakes, please. Texas Employer New Hire Reporting Form Please complete neatly. No mistakes, and ALL CAPS. If a mistake is made, please print another copy, and complete it over again. (Complete #14-25) MISD Direct Deposit Enrollment Form Print employee name, check the box, enter the bank name, routing number, account number, sign and date. A voided check or a bank’s direct deposit form must accompany this form before it is turned in to Human Resources. Form W-4 Complete the bottom portion of this form, and sign it. (#3 and #5 must be completed) A social security card is required with this form. You will not be paid unless it is provided. No mistakes please.

6 Paperwork continued… PARAPROFESSIONAL & AUXILIARY STAFF ONLY
TIME CLOCK RULES All paraprofessional and auxiliary staff must clock in and out daily. This includes clocking in and out for lunch or going off campus for personal reasons.

Congratulations! you Should have received or will be receiving your contract electronically. Please sign and return it back to Lupe Castaneda, Assistant Director of Staffing, at within 2 days of receiving it.

Letters of Reasonable assurance: Paraprofessional & Auxiliary staff only Congratulations! You will receive your letter of reasonable assurance when you turn in your paperwork with human Resources.

9 Next steps… Now that you have completed your paperwork, MISTAKE-FREE, please bring your paperwork to the Human Resources office between the hours of 8am - 12pm on Fridays only. When you Drop off your paperwork, you will need to bring the following: YOUR PAPER work, and the acceptable documents listed on the I-9. (I.E. SOCIAL SECURITY CARD and Driver’s License) READ ALL YOUR Attachments IN THE . Please sign up for your benefit session with Mr. ezequiel Zaragoza, Benefits Coordinator. All employees must attend a Benefit session to sign up or decline health coverage within the first 30 days of employment. No exceptions will be made.

10 Continue: Salary schedule : TO determine your pay for the school year, please review the attached schedule. (It has not been updated for the school year) *If you begin the school year after the first day of your contact, your pay will be prorated based on the number of days you will work. Ex. Salary will be determined based on number of days In your contract. EX: First Year Teacher Salary: $44,500 divided by 12 months = Your monthly Salary ; $44,500 Divided by 187 days = Daily Rate ( If you work less than 187 days, you will get paid for those days worked based on your daily rate and then divided by the months left in the school year) Service Records (See attachment) – You will need to turn in an original service record to receive years of credit. It may be best to request that it be sent to you directly to ensure you receive it. After you receive it, you can submit it to HR for salary adjustment. Master’s Degree FOR Teachers only – Receive $ You must submit an official transcript to receive this incentive.

12 About your technology:
All professional staff who join the MISD family will receive a laptop and an IPAd. The technology devices will be received during the onboarding session on Wednesdays from 8 am – 12 pm.

13 Compliance Videos: You will be required to complete your compliance video training within 30 days of date of hire. See attached .
