1 EARLY FIELD EXPERIENCES GUIDELINES 2012/2013 Presentation Prepared for Courses Requiring Field Experiences by Gwendoline Ayuninjam, Ph.D. Director of Field and Clinical Experiences (OFCE)

5 Required Forms and Documents 1. SEHP Early Field Experiences Placement Request Form 2. SEHP Employment and Placement Confirmation Form (if applicable) – For candidates employed in eligible sites (i.e., classrooms or schools) where field experiences may be completed. 3. Proof of Current Professional Liability Insurance coverage 4. Background Check (Follow host school’s guidelines)

6 SEHP Early Field Experiences Placement Request Form All candidates requesting placement must complete, sign, and submit this form to OFCE in AC 217 Submission Deadlines: *Fri., Sept. 7, 2012 (Fall 2012 placements) *Fri., Feb. 1, 2013 (Spring 2013 placements)

7 SEHP Employment and Placement Confirmation Form (if applicable) If employed in a classroom or school approved by your course instructor for field experience completion, a fully completed and signed Employment and Placement Confirmation form must be submitted with your Early Field Experience Request form for your request to be honored.

8 Proof of Current Professional Liability Insurance coverage All candidates are required to provide proof of current professional liability insurance (NOT collision) coverage. 9 Background Check Follow background check process for host school or district, e.g., Winston Salem/Forsyth County requires completion of volunteer background check through the district’s website. This information will be communicated to you along with your placement confirmation via email. Note: You will need the exact name of your host school in order to be able to complete the volunteer background check process.
