1 Manchester Children’s Community Nursing Team – Who are we? and what do we do?

2 When did it all start? First UK children’s community team formed in 1949. Manchester team formed in 1984 by Peggy Keating. Manchester’s was one of the first children’s community nursing teams. Initially based in north Manchester, later became citywide.

11 Referral by phone: NURSING TEAM & SPECIALIST SERVICES 0161 248 8501 (24 hr answer phone) Referral by FAX NURSING TEAM & SPECIALIST SERVICES 0161 248 6267 Referral by email ccnt@cmft.nhs.uk Referral by post: Children's Community Nursing Team Longsight Health Centre 526-528 Stockport Rd Manchester M13 0RR We endeavour to respond to any referral within 2 hours (within operational hours) ALL URGENT REFERRALS SHOULD BE MADE BY PHONE Child is a Manchester resident Under 16 years old (19yrs with Special Needs)

12 Referral Pathway to CCN team Referral checked by co-ordinator. Child lives in Manchester Area. Child is aged 0-16 years. Child has a medical need. Child’s condition is most appropriately managed by CCN team, not another agency. All relevant info is on referral. Co-ordinator then determines care needed by team and when this should be provided. Contact is made with family to arrange either a home visit or clinic appointment.

13 Children’s Community Nursing Clinics Offer: Clinical assessment of acute and chronic diagnosed conditions Administration of medication in line with CMFT & NMC guidelines, via prescribed route this may be inhaled, oral, gastrostomy, subcutaneous or intravenous. Advice and education in the management of the child’s condition Wound Care & Dressings Flushing of central lines and devices Monitoring of oxygen saturations, Injections Subcutaneous and intra muscular

16 Children’s Asthma Service "Delivery of a high quality of care, management, educational and support package in line with best practice to children and their families with asthma".

17 Service Provided by Asthma Team Home Visits Appointment Clinics Longsight, Charlestown, Northenden, Wythenshawe Drop-in Clinic Each Thursday 2-4pm at Longsight Health Centre.

18 Reason For Referrals To Asthma Service Repeated oral steroids Repeated attendances/admissions to hospital. Repeated GP/PN visits Uncontrolled symptoms Concerns Referrals should be made to the service from the health professional/GP surgery.

19 Children’s Phlebotomy Service City wide clinics Run by band 3’s and band 4’s Overseen by band 6 sister Referrals from GP/Consultant Very popular service! Professionals will signpost to this service once it is indicated that a blood test is needed. A blood form must be given to the family as well as Ametop prescribed if needed. Appointments are made by phone, usually by the child’s parent/carer. Tel: 0161 248 1229.
