2 2 Definition of Palliative Care n n Active total care of patients whose disease no longer responds to curative treatment, n n Focusing on quality of life and n n Integrating the ─ ─ physical ─ ─ psychological ─ ─ spiritual ─ ─ social aspects of care

3 3 Palliative care n n Cancer and non-cancer n n requires a collaborative (multi-professional) approach n n follows the patient: Provided at home, in nursing homes, in residential care, in hospitals and in specialist units

4 4 The palliative care approach n n is good clinical practice n n informed by a knowledge and practice of palliative care principles n n should be a component of health care available to all patients who need it

5 5 Hospital Support Specialist Palliative Care Team n n 3 Specialist Palliative Care Nurses n n 2 Consultants (sessional input) and 1 SpR in Palliative Medicine n n 2 Specialist Occupational Therapists n n 1 Palliative Care Social Worker n n 2 Specialist Physiotherapists The team is based in New Cross Hospital and supported by a Team Co-ordinator

6 6 Why refer to the specialist palliative care nurses? n n Advice: – –For pain and symptom management – –If complex psycho-social issues identified n n Help with planning a complex discharge n n Identification of patients who require specialist in-patient care within a hospice setting

7 7 Why refer to the palliative care social worker ? n n Patients with palliative social care needs: – –To plan and organise social care after discharge – –To assist with the transition into palliative care nursing homes – –For financial advice for both patient and carers

8 8 Why refer to the specialist OT and Physiotherapist ? n n Give patients some autonomy back n n Active rehabilitation may be key to best quality of life n n For preparing a successful discharge home with appropriate equipment n n Guiding family on practical issues for future care

10 Services provided by Compton Hospice n n Home Care and Hospice at Home n n Day Care n n In-Patient Admissions for: – –Pain & Symptom Control – –Psychological Support – –Respite – –Rehabilitation – –Terminal care n n Bereavement Support

11 11 In-Patient Admissions at Compton Hospice n n 22 Beds for catchment area of over one Million Population n n Average Stay 14 Days n n Waiting List operates – –Highest needs for Specialist Care take priority n n 60% Discharged, 40% Die in the Hospice
