4 COAST An acute community children’s nursing team Avoid a Hospital admissionOr help get a child home more quickly from hospital7 days a weekThe GP can refer a child ifPorts City, Fareham & Gosport or East Hants GPOne of 10 conditions, eg. asthma, gastroenteritis, chest infectionLess ‘quality’’/general follow up referrals, more avoided admission referrals
5 Children and Young Person’s Therapies Provides an integrated therapy service:Occupational TherapyPhysiotherapySpeech and Language TherapyAccept referrals of children who:Are aged 0 – 19 yearsHave a Hampshire, Southampton or Portsmouth GP or who attend a Hampshire, Southampton or Portsmouth school.Present with a complex or uneven profile of developmentChildren whose needs cannot be met by those who work closely with themWorking with Parents – the experts in their children’s care.Working in homes, schools, clinics – wherever the child is.
6 Muscular Skeletal Services (MSK) – Physiotherapy and Pain Our patients are adults over 18Receive referrals of more complex patients from Southern Health’s physio service.Treatments include:PhysiotherapyJoint injectionsIndividual and Group treatmentsService can refer onto QA for surgeryPain serviceFor persistent pain (typically over 3 months), the service supports through group sessions to support self managementBased at Crofton Centre, Stubbington
7 Podiatry Foot and Ankle conditions Sometimes Diabetes linkedFoot UlcersNail surgery requirementsProvided at various locations across Hampshire and PortsmouthDomiciliary service available for housebound patients.Close working with QA for acute issuesService does not cover routine nail care which is not provided by the NHS, but is available as a paid for service
8 Special Care Dental Service Our patients are children and adults who have particular needs:a physical, sensory, intellectual, mental, medical, emotional or social impairment or disability or a combination of these which may prevent them from accessing care through the General Dental ServicesSpecialist occasional care, supporting care in general dental practice by providing a single or staged course of treatment.General Anaesthetic services from Poswillo on the QA siteDental surgery services from 18 locations across Hampshire.Domiciliary care services in people’s homes and nursing homes.
