Student Registration Card 4 All 8 th graders are required to have one semester of PE or a year of athletics. 4 Academic teachers have recommended next year placement. 4 If you are requesting a change in placement, circle and initial the choice.

Athletics... 4 Athletics lasts the entire school year. 4 If you do not make a team you will be in off season which involves conditioning and running. 4 A Physical Night will be held later this school year.
More on Athletics 4 You are required to dress out in a KMS athletic uniform & appropriate shoes 4 The KPHS coaches like to see middle school students participate in all sports 4 In addition to a physical form you also need to have a birth certificate 4 For security purposes you must have your own lock and be responsible for locking up your items in the locker room

This locker room is used throughout the day by all students. Valuable items need to be locked up after students dress out for PE and leave the area.
