Dress CodeStudent wears clothing that is not within the dress code guidelines defined by the school/district. Non-serious, but inappropriate physical contact. Student pushes in line, shoves, minor/aggravating tapping, nudging. Non-complianceStudent displays brief or low intensity failure or hesitation in responding to adult requests. Inappropriate Language Student engages in name calling and/or use of words in an inappropriate way. LyingStudent delivers message that is untrue. Missing or late Homework Student does not turn in his/her homework assignment or homework assignment is late or incomplete. OtherStudent engages in any other minor problem behaviors that do not fall within the above categories. Minor Infractions Teacher/Classroom Managed Behaviors Consequence: Four (4) minor behavior infractions constitute a major infraction, which results in an office referral. Teachers will document all minor behavior infractions and contact the parent as necessary.

Disrespectful messages include negative comments based on race, religion, gender, age, and/or national origin. Major DisruptionStudent continually yells, screams, makes noises, roughhouses and/or is out of their seat. CheatingStudent deliberately violates rules. DefianceStudent intentionally ignores a staff delivered directive or knowingly breaks a school rule. Vandalism / property damage Student participates in an activity that results in substantial destruction or disfigurement of property. WeaponStudent is in possession of knives or guns (real or look alike), or other objects readily capable of causing bodily harm. DrugsStudent uses/possesses controlled substance; violates medicine policy (OTC or prescription). OtherProblem behavior causing this referral is not listed above. Staff using this area will specify the problem behavior observed. Major Infractions Office Managed Behaviors Consequence: These consequences may include but are not limited to a parent contact, detention, in-school suspension, and/or out-of-school suspension. B

14. New Referral for PBIS Assistance Form  If you need suggestions/help with a student’s behavior, simply complete new form!  Located on share drive in PBIS folder.  Fill it out and turn in to E. Godwin.  Meeting will be scheduled to discuss the referral and come up with a plan.
