1. Click ‘Sign up or sign in’ 1 Go to the web address www.healthvault.com

2. 2 Click ‘Sign up now’ at the bottom

3. 3 On the Create an account page, complete the following info: Enter your First and Last Name Create password Enter your password again Enter your zip code Enter your birth date Select your gender Enter your personal e-mail address

4. 4 Enter your phone number Enter the characters you see (the characters you see on screen will be different than the ones on this page) Uncheck the box Click ‘Create account’ Continue creating your account by entering the following info

5. 5 Go to your Yahoo! or Gmail account. Look for the ‘Microsoft account team’ email and open it. Click the blue ‘Verify…’ button inside the e-mail you just opened. After clicking the Verify button, you will be automatically taken back to your web browser. You will see a message from Microsoft account. Click ‘OK’. Microsoft will automatically send a verification e-mail to the account you entered earlier. You do not need to do anything on this screen.

6. 6 The next screen shows the information you entered when you created the account. Click ‘I accept’

7. 7 In the gray box on the left side, click on ‘Health messages’

8. 8 Click ‘Allow access’ at the bottom. This page allows you to give other users access to your health information if you choose to in the future (parents, siblings, children, etc).

9. 9 HealthVault assigns you a Direct email address. Independence Center needs to enter this email address into CareLogic (CLIC) in order to send health information created within CLIC to your HealthVault acount. Click ‘Continue to inbox’

10. 10 Your inbox looks like the screenshot below. This is where Independence Center will send your health documents using the Direct email address assigned to you. If you have access to a printer, click the PDF link above to print a letter explaining you have created a HealthVault account and would like your health information sent here. Bring this letter to your next appointment so that we may keep a record of your Direct email address.

11. 11 This is what the letter will look like. You can also give this letter to your other doctor’s requesting that they send health documentation to your HealthVault account.

12. 12 Take some time to explore HealthVault. Use the menu on the left to navigate the pages of HealthVault. You will find any health document that Independence Center has uploaded to you page under ‘Documents’. Click the ? for help topics To EXIT HealthVault, click the arrow next to your name. Then click ‘Sign Out’ Try the ‘Let’s get started’ box to start entering basic info about yourself.

13. 13 You will have two additional e-mail messages in your Yahoo! or Gmail account welcoming you to HealthVault and to the HealthVault Message Center When you return to HealthVault the next time, you will log in using the Yahoo! or Gmail address and password you used when creating your account.
