My Care Pathways Create increased patient commitment –New information and communication channels –Individualized patient information –Inviting new players in the market –Encouraging co-production –Encourage new services, notably health status and nutrition Increase the patient’s empowerment –Improved transparency in health care processes –Let patients administer, own and manage their own processes Increase the safety of health care –Reduces the risk of mistakes

My Care Pathways Focus on four health care processes, relating to: –Stroke – Stockholm County Council –Lung cancer – Stockholm County Council –Hip surgery – Skåne Regional Council –Heart disease – Region Västra Götaland Develop methods and guidelines –Describe the development of other health care processes and ancillary services –Co-production with trade associations representing health care, industry and the patients. Make it possible to solve problems together Stimulation for the development of new products –Enablement of a new service sector, both inside and outside the county council
