How to access Rewards Program Descriptions Wellness Profile The comprehensive health risk assessment helps members understand their health status and gives you a personalized action plan to improve or maintain your current health. Symptom Checker This unique interactive feature makes it easy to learn what to do about symptoms and when to contact a doctor. Click on the part of the human body graphic where your discomfort or symptoms exist. Health Education & Information With news articles, a health library, condition guides, e-newsletters, videos, and links to relevant health information, research a specific health topic or get the latest info on health trends. Personal Health Record With the secure online personal health record (PHR), store and access health information anytime, anywhere. Grant access to health care providers and family members to facilitate treatment, for example, in emergencies. Track medications, immunizations, medical exams, treatment history and other health information for yourself or other family members. Health Trackers You can choose from 24 different trackers to record and view your progress in key health measures such as blood pressure, blood glucose, cholesterol, and weight. Digital Health Assistant Programs Learn about programs offering real life solutions to everyday wellness challenges. Topics include exercise, nutrition, stress management, weight management and emotional health. Health Management Centers These proven resources help members manage health issues, such as exercise, weight management, nutrition and stress. Tobacco Attestation This option gives credit to individuals who are tobacco free or working towards becoming tobacco free. You can certify that you are tobacco free or have/will enroll in an approved tobacco cessation program to earn 10 points towards your Rewards program completion. Highmark Wellness Rewards motivates employees to take an active role in their health by offering them rewards for making healthy choices. The points-based structure enables you to choose from a variety of activities, each with their own point value. As activities are completed, points accumulate towards earning rewards.
