5 SERVICE INTEGRATION Survey on levels of collaboration developed and distributed in consultation with Health Nexus Conducted review of Service Coordination Guidelines to identify best practices/barriers/training needs Aligned SC Guidelines with work of Special Needs Strategy Shared SC Guidelines with Lanark Planning Table Expanded committee to broader planning table

6 SERVICE INTEGRATION Child and Family Centres Rideau Child and Family Hub [pilot] -planning for September launch -coming soon info being distributed -working on logic model

7 TRANSITION TO SCHOOL Coordinated planning and delivery of Welcome to Kindergarten events in all LG schools (community partner representation at all schools) Ongoing work with school boards to build on effective parent engagement practices in terms of transition to school (Regional Transition to School workgroup)

8 DEVELOPMENTAL SCREENING Recommendations 1. Professional Development focusing on : Sharing Sensitive News, Community Agency/Program Awareness and Facilitating Conversations with parents 2. A link/page be added to the EKIOC website that links to the electronic portal of the NDDS 3. Provide opportunities for regular access by families to Nippissing Screening tool.

9 LITERACY Infant literacy “tubs” developed to reach families of infants/toddlers through libraries Plan to evaluate the impact of Tubtastic pilot initiative in terms of increased capacity of librarians to work with the birth to 3 population and their families Continue to provide deliberate support for more vulnerable families through outreach to community organizations with whom families are connected. (e.g. food banks, Loaves and Fishes etc.) Read to Every Kid

10 DATA Plan for data collection and community data profile developed. EDI results and information on sub domains shared with community partners Health Nexus Survey completed and shared Plan to provide training on survey development

11 SPECIAL POPULATIONS 1. French Language Population Resources shared on EKIOC website. Working with French language BSN to support outreach to communities in Leeds- Grenville Attended WTK at French schools French language support is a standing item on BSN agenda 2. Aboriginal Population Resources posted on EKIOC website Yearly plan to support aboriginal population

12 EMERGING PRIORITIES Community response to “How Does Learning Happen?” framework Developing strategies to engage hard to reach families Community response to government initiatives (e.g. Child and Family Centres, early years mental health policy) Continued work with school boards to implement effective Transition to School practices) Family Engagement – deliberate strategies for expanding/supporting Support for Special Needs Strategy Analysis of Health Nexus Survey results – how we are working together, what collaborative relationships need to be strengthened To Do this we need consistent, committed membership.
