1 Amendments in ACAP © Created by Isaac P. E. Mailleue Research Integrity Coordinator University of North Carolina at Greensboro September 2013

2 Amendments in ACAP No one can see the future. If you realize you should have added another student or want to use additional animals after you have submitted and received IACUC approval for your protocol, you will need to complete an amendment.

3 ACAP developers and the ORI recommend you use Internet Explorer when using ACAP. Some bugs have been much more likely to occur with other browsers. We also recommend you always click buttons instead of hitting the “Enter” key. Again, some bugs seem more frequent if you hit the “Enter” key

4 Log into ACAP If you would like to make changes to your protocol after IACUC approval, click “Approved”.

5 Now click on the study you would like to make changes to.

6 If you want to change the title, PI, or funding agency for your study, click “Title/Funding Changes” For any other changes, including personnel additions, click “Amendments”

7 Clicking “Title/Funding Changes” will allow you to change the title, PI, or even your source of funding

8 For all other amendments, click “Amendments” then “Add Amendment to Application”.

9 Once you have clicked “Add Amendment to Application”, be sure to name your application. A basic description is helpful (i.e. “Additional Personnel”, “Additional Animals”, or “Changed Sedative”) Now click “Start Amendment”

10 A pop-up window will display the following text. This is normal.

11 Click on the section you would like to make changes to. For this presentation, I want to add an additional animal Therefore, I picked List Item 4. “Reduction, Refinement, Replacement, and Animal Numbers”

12 Then I went to the appropriate items (4.2 and 4.3) to make changes I gave a brief description of the change for 4.2 I also changed the animals listed in 4.3 Save changes to return to the Item List

13 Note that “Amendment Information” does not have a check. Click on “Amendment Information”

14 Make sure Item 1 (Amendment Title) is filled out, then select “Click to View/Edit” Please provide a summary of what you have changed in this amendment and provide the item number of the changed item.

15 List your changes in the pop-up. Then click “Accept Changes”

16 You should now have a check beside “Amendment Information”

17 Clicking “Preview Amendment” will open a new tab or window This new tab will display your modification of the original protocol. After verifying that all your desired modifications have been made, return to the main ACAP tab.

18 Make sure all other lines have checks, then click “Submit Amendment”

19 You will need to certify that you have read and agree with the policies of UNCG, just like you do for a new submission.

20 Take note! You will NOT be able to make further changes after you submit. If further changes are required by the IACUC, the ORI can change the submission status of your protocol, allowing you to make those changes.

21 Congratulations! You have submitted your amendment on ACAP!
