Engagement of Community Residents Development of Community Leaders Collaboration among Community & Organizational Partners Bridge-building with External Decision Makers Engagement of community residents in decision making, including identifying an issue of concern, mobilizing to take appropriate action, and providing support for ongoing involvement in their communities; Development of resident leaders who have the capacity to assume leadership roles in their communities and to represent their communities to external decision makers; Collaboration among community and organizational partners, to work together and with community residents to develop and implement a work plan for the project; Bridge building with external decision makers (such as city council members, school board members), to develop relationships and interest to support the community’s approach to addressing the identified problem.

It is expected that the CBI Model will facilitate the development of community capacity to make measurable change on an identified problem. Briefly, the key activities associated with each phase of work include: Community Problem Selection Process: Steps– to be completed by April 2014 Community Assessment Identification of 1-2 problems for further analysis Problem Analysis for 1-2 problems Selection of one community problem for the CBI 2) Plan for Implementation – Initial elements included in RFP Steps to be completed by July/August 2014 Convert problem statement to goal statement and identify outcome measure Identify strategies and strategy measures Identify baseline measures 3) Implementation Implementation grants awarded -- Sept 2014 Steps Select activities Select targets associated with identified measures Provide resident training Implementation of activities Both the CBI pillars and the phases of work are deeply grounded in the evidence on best-practice.
