The focus of this particular slideshow is Section 3 – “WCB Statistics” City of Fort Saskatchewan Safety Qualification Form Thank you for choosing to work with the City of Fort Saskatchewan! This slide show is intended to help walk you through filling in the form and providing examples of what attachments will be required.

3 Section 3: WCB Statistics
If you are an employer in an exempt category, attach your WCB clearance letter which will state that you are exempt. Despite being exempt, all employers performing work for the City of Fort Saskatchewan must obtain and maintain WCB coverage for the duration of City projects. This includes coverage for owners, where owners are involved in work site activities. NOTE: A recent (within 1 week) copy of the WCB Clearance letter addressed to the City of Fort Saskatchewan must be included in the safety qualification package. After that we will add you to an automatic clearance list. Section 3: WCB Statistics Does the company have a WCB Account in Alberta? WCB STATISTICS Does your company have a WCB account(s) in good standing? Yes No (if yes, please attach a current WCB clearance letter addressed to the City of Fort Saskatchewan) WCB Stats from the last 3 years 20____ Employers premium rate Industry rate Rate adjustment, surcharge or discount Number of fatalities Number of Lost Time Injuries* *On a separate page, briefly explain any fatalities or lost times that may have been listed. Attach a copy of the current year WCB Employer Premium Rate Statement and the two previous years. Employers operating in Alberta should have an active WCB account. There are exceptions to this such as certain industries and owners of businesses.

This portion requires you to refer to your Premium Rate Statements for the past three years. If you already have these, click here. If you are not sure how to obtain these, click here. 5 You will need to go to the WCB Alberta webpage and create an account with “myWCB”….
…..click the link here…..

6 …or information from your last WCB return.
Questions will come up during the process of setting up your account. …and you come to the MyWCB page. You will want a few documents available such as your last invoice....

7 Create your User name and password…

8 …click on your position...

9 Found here. …the box will expand to collect WCB account number (from the invoice page)… …and then select the category of user.

10 Answer any one of the questions and complete the registration process.
When all the registration is complete, you will be allowed to login to the system.

11 Once your “myWCB” account is crated and confirmed, login to your account on the home page.

12 Select the “My Claims and Costs” Tab….

13 … and then select “Request Claims and Premium Reports” from the drop down menu.

14 You will have to request the report(s) you want
You will have to request the report(s) you want. In this case, “Premium Rate Statements”

15 When the next year Premium Rate Statements become available, please remember to forward those off to us as well so we can keep your file current. Select your account number…. …and then select the year you want. At a minimum the City will want the current year and the previous two years as well.

16 Within 24 hours you will have copies ed to you and they will be now available through your myWCB account.

17 Section 3: WCB Statistics
The Premium Rate; The Rate Adjustment. This may be in a Surcharge or at a Discount. Using the Premium Rate Statements, fill in the following information for three years… The Industry Rate; Section 3: WCB Statistics Does the company have a WCB Account in Alberta? WCB STATISTICS Does your company have a WCB account(s) in good standing? Yes No (if yes, please attach a current WCB clearance letter addressed to the City of Fort Saskatchewan) WCB Stats from the last 3 years 20____ Employers premium rate Industry rate Rate adjustment, surcharge or discount Number of fatalities Number of Lost Time Injuries* *On a separate page, briefly explain any fatalities or lost times that may have been listed. 18 Section 3: WCB Statistics
… and write a brief summary about every incident that is identified. Do not use names but describe: What happened; Why it happened; What corrective measures the company is taking to prevent it from happening again. Include this on a separate page to allow proper space for details. Fill in any fatalities or Lost Time Injuries (LTI’s) that the company may have had for those years… Section 3: WCB Statistics Does the company have a WCB Account in Alberta? 19 Section 3: WCB Statistics
Attach copies of the current year Premium Rate statements and the past two years. This means that if it is currently 2018, you should attach the PRS’s for 2016, 2017 and 2018.
