2 Why Wellness? Appriss firmly believes in creating a work environment that allows our top talent to operate at their best. This work environment extends beyond our new workspace, and even beyond our policies and procedures. It has to be core to what we do and our values. Providing great health and wellness benefits is another element of our commitment to top talent. 4 Current State vs. Future State
Our current wellness model is almost entirely built around tobacco usage.   We spend well over $1 million dollars paying non-smokers... to not smoke. Employees and Spouse active on Appriss Medical 9/1/18 wanting lower premiums in 2019 Note: Employee and Spouse are not dependent upon each other for participation incentives If you are a new hire becoming eligible or enrolling in Appriss Medical after 9/1/18, your program will be the same but will begin 1/1/19  Pregnancy Alternative Program within SimplyWell Portal requires appeal form to be submitted

8 Biometrics partnership with Quest
Quest Portal (Sign ups coming Early Oct.) Onsite Events (Appointment Required) Linn Station (October 25, 26, 27, 29, 30) Torrance (October 29) Irvine (October 29) Quest Center (Registration required, appointment preferred) Primary Care Physician* *Cost associate with this visit will need to be submitted to insurance as it will not be reimbursed by Appriss.  You will also be responsible for submitting form online.
