2 Held in December 2014 Tables set up in training room by station Employees registered and completed Health Assessment Survey Screened on-site for Height, Weight and Body Mass Index (BMI) Blood Pressure Sugar/Cholesterol test by finger stick Review results with health coach Results also sent to employee portal for further analysis and personal recommendations. Biometric screening Implementation of new Corporate Design - March 2015

4 Held in February 2015 Tables set up by vendor Had multiple drawings, items provided by vendors Employees had card signed by each vendor to be eligible for the drawings. 6 On-site bus for mammograms (breast cancer screening) Open to all female employees and spouses covered by medical insurance. Covered 100% by medical insurance for preventive screening On-site Mobile Mammography Implementation of new Corporate Design - March 2015

7 Recognize years of service in 5 year increments Hold a luncheon in the training room Employee received lunch, recognition by supervisor and plaque Service Award Dinners Implementation of new Corporate Design - March 2015
