3. Design and planning phase
(1) Decide on type of survey (e.g., mail, telephone interview, etc); Type of respondent , and population (2) Develop the survey instrument/ questionnaire - Write questions to measure variables - Decide on respondent categories - Organize question sequences - Design questionnaire layout

4. (3) Plan System for recoding answers (4) Pilot test the instrument and train interviewers if necessary (5) Draw the sample: - Define target population - Decide on type of sample - Develop sampling Frame - Decide on Sample size - Select Sample

5. Phase.2: Data Collection
Locate and contact the respondents
Make introductory statements or provide instructions
Ask questions and recode answers
Thank respondent and continue to next respondent
End data collection and organize data
