3 Background RFB: RAF/2012/00029: Appointment of a service provider to provide Funeral Benefits Scheme for the Road Accident Fund (RAF) employees for a period of five (5) years. Advertised on 30 November 2012 Closing on 21 January 2013

4 Background The RAF wishes to appoint a suitably qualified Service Provider to provide a Funeral Benefits Scheme for RAF employees for a period of five (5) years.

6 Evaluation Criteria Evaluation will be based on the following:
Technical Functional Mandatory (Comply or Not Comply) – Bidders who do not meet the mandatory requirement will be disqualified. Technical Functional Requirement (100 points) Price and BEE Evaluations (90/10 points)

7 Evaluation Criteria Mandatory requirements
The bidder must be a licensed service provider with the Financial Service Board (FSB). Please furnish the RAF with a valid license certificate from FSB to confirm that you are licensed with FSB.  The bidder must be primary services provider to provide with the Funeral Benefits Scheme. Please furnish the RAF with proof that bidder is the primary service provider to deliver the required service. Failure by the Fund and the successful bidder to conclude a contract within 60 days following the issuing by the Fund of the award letter to the successful bidder, will result in the bid being deemed to be non-responsive which will then entitle the Fund to consider other responsive bids and award the tender to another bidder or bidders.

9 Disqualification of bids
Bidders shall be disqualified if they fail to: Submit a valid and original tax clearance certificate Complete questionnaires in full Submit true and correct information Comply with mandatory requirements Comply with prescribed response format

10 Submission of bid responses
Original signed bid document and Proposal clearly marked and indexed with all pages numbered One original and two (2) hard copies, submitted in a sealed envelopes. Submission Address: RAF Menlyn Reception 38 Ida street Menlo Park Pretoria

11 Submission of bid responses
Bid responses sent by courier must reach the reception at least 36 hours before the closing date to be deposited into the bid box Submission Register must be signed at the reception by bidder when submitting bid documents. Important note: Please ensure that the attendance register has been signed Name of company Contact details Late bids will not be considered

13 Purpose of the RFB The RAF currently has a Funeral Scheme in place;
The benefits by the current service provider do not cover parents and parents-in-laws above the age of 75; The purpose of the RFB is to improve on benefits by the current service provider and also include acceptance to the scheme for parents and parent-in-laws who are older than 75.
