3 Details of the Bid REQUEST FOR PROPOSAL:ELECTRONIC FORM SOLUTION Compulsory briefing session 23 July 2013. Published 12 July 2013 Closing date 06 August 2013. All questions / enquiries outside this forum will be via email to: tsholok@raf.co.za not later than 11:00 on 26 th July 2013 tsholok@raf.co.za Questions/ Answers will be published on the RAF Website on 29 th July 2013.

4 BACKGROUND Road Accident Fund (RAF) wishes to develop an online claims e-form. The RAF therefore, requests proposals from bidders who are able to develop and provide the E-forms Solution that will allow claimants to capture their claims online.

5 Compliance Compulsory Documents (Valid & Original Tax Clearance Certificate) Page 5-Checklist of documents to be submitted Please ensure that the bid document is signed where required Completed bid and response to evaluation must be submitted

6 Evaluation Criteria Mandatory Requirements The following specific requirements must be met by the bidders and it will be expected of bidders to supply proof or confirm their commitment where applicable. The bidder must provide a Project Manager with a relevant Project Management Qualification. (Kindly attach proof of qualification) The bidder confirms that it has read and understands the terms and conditions set out in the attached Service Level Agreement (SLA) (Annexure J) which terms and conditions the bidder accepts, in the event that the RAF awards a contract to the bidder pursuant to this bid. The Initialled Draft SLA by the bidder must be attached to the bid response in duplicate. (please mark the appropriate box) Failure by the RAF and the successful bidder to conclude a contract within sixty (60) days following the issuing by the RAF of the award letter to the successful bidder, will result in the bid being deemed to be non-responsive which will then entitle the RAF, at its sole discretion, to consider other responsive bids and award the tender to another bidder or bidders. (please mark the appropriate box) NB:FAILURE TO COMPLY WITH ALL OF THE MANDATORY EQUIREMENTS WILL LEAD TO DISQUALIFICATION

7 Technical / Functional Requirements The technical criterion that will be utilized to test the capability of The service providers please see circulated copy for details. Technical criterion will be evaluated against the following detailed requirements: The bidder will be evaluated and scored on five elements with a total of 100 points, Threshold: All bidders who score 60 or more out of 100 points will be shortlisted for Price and BEE.
