Isolate batteries before attempting to handle
INCIDENT Rig electrician was attempting to place a rubber mat under the 2 x 12 v batteries of engine #1. The chief electrician was standing nearby. A +ve terminal came in contact with lube oil filter creating a electrical short circuit which led to an explosion and fire. This resulted to severe burns to the electrician, who later died, minor burns to the chief electrician and damage to the engine. ROOT CAUSES: Use of batteries of the wrong specifications in the wrong position. Poor hazard identification. Poor implementation of PTW and TRIC. Poor supervision (communication) of rig maintenance activities. Lateral Learning Don’t change battery specifications or orientations. Battery hazard awareness (terminal leads fully covered). PTW system must be applied for any job requiring electrical or mechanical isolation. Even for 12 v batteries. Hold formal daily meeting with key personnel for planning all activities (PTW/TRIC planning).
