HSE LEARNING INCIDENT: Four LCC contractor staff were involved in RTA rollover at a bend on a graded road in the Rima – Mukhaizna area. The driver was only 2 days with the company and did not have a PDO, Defensive Driving permit. All four occupants of the vehicle were thrown out. One of them died in hospital. There was no working speed limiter or IVMS in the vehicle. LEARNINGS: Company: Do not permit a driver to drive when he does not have PDO DD training. Do not allow a vehicle to be used without a working speed limiter or IVMS. Driver: Comply with PDO’s speed limits for graded and blacktop roads. Driver & Passengers: Wearing seatbelts saves lives. All: Intervene when safety rules are broken (You may save lives). They are Life Saving Rules……. Follow them!
