1. INCIDENT:
A 50 T crane was lifting pressure equipment skid (2.5 ton) from the wash bay area to
wellhead equipment area. The crane was positioned in between the two areas and
crane operator was instructed to move the load, the left hand out riggers had not
been extended. The crane lifted the skid and made a 180 degree turn, the
momentum of the load caused the crane to topple over to it’s left hand side. As a
result of the accident, the crane boom crashed through the roof of the iron inspection
area where work was ongoing, the worker inside suffered head injuries. The crane
banksman twisted his ankle while he was running away from the scene.
CAUSES
No TBT conducted to ensure all hazards are covered during the discussion.
The operator did not apply the crane operating procedures. (Outriggers not extended prior starting lifting operation
No observation / intervention practice by the crew.
LEARNINGS
Conduct pre-job meetings, discussions for the job without short cut, PTW, TBT, JSA to cover all risks (documented)
Ensure the outriggers are fully extended no matter how small is the load.
Clear all proximity area before any crane operation.
Stop the job Obligation must be implemented by Anyone who recognizes a flaw in the operation
Recommended Actions:
Discuss this alert in toolbox talks and HSE meetings and
Distribute to all contractors
Post on HSE notice boards
Include in site HSE induction
Ensure the outriggers are fully extended, no matter how small or light the load
