INCIDENT The 13 3/8” coated casing had been run and the IP (floor man) was making up the surface cement lines. One side of the Chiksan had been installed. While attempting to make up the other end, IP's right hand got trapped between the Chiksan and Y-junction piece. He suffered harm to his ring and middle finger. ROOT CAUSES: No Effective Toolbox Talk. Improper Manual Handling. Poor hazard identification.. Lateral Learning Plan the work and think of potential hazard. Do not place hands in a pinch point. Plan lifting jobs properly. Supervisors to be in control. Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Distribute to all contractors Post on HSE notice boards Include in site HSE induction Key Learning: USE WINCHES INSTEAD OF HANDS AND FINGERS!
