INCIDENT
The Instrument Technician was working in the Auxiliary Area of the Qarn Alam Steam
Project CCR. At around 0840hrs he went to the exit door to look for the tea pickup,
and saw some workers gathering outside. Because use of this door was not permitted,
He turned to exit the building via another door. In doing so he stepped into a floor
opening nearby. This resulted in a fall and fracture injury to his left leg.
ROOT CAUSES:
Insufficient control of work site layout to minimize hazards (eg storage area too close to active work)
Lack of reinforcement of the intervention culture at this location (STOP Cards available in control room but little used)
Lateral Learning
Do not enter an ‘active’ work area unless you have work to do there
Provide proper barriers where required around ‘active’ work areas, and keep stored materials out of the way
Do not stand on construction materials & equipment
Provide temporary covers when floor panels have to be removed for prolonged periods
Recommended Actions:
Discuss this alert in toolbox talks and HSE meetings and
Distribute to all contractors
Post on HSE notice boards
Include in site HSE induction
