6 Submission of Proposals
Proposals are due January 31, 2017 at 4:00 pm Delivered to Procurement & Strategic Sourcing, 5700 Cass Avenue, 4th Floor – Suite 4200, Detroit, MI 48202 We require an original plus one copy (2 total) of your proposal. An electronic version is required, which should be submitted to Proposals are limited to 35 pages total. Prices and other requested data must be on the Schedules Provided Proposals must be responsive and satisfy all mandatory requirements Any deviation from requirements must be noted in Exhibit 1 as Restricted Services

9 Vendor Presentations Selected VENDORS may be invited to make a presentation at a set time and date Presentations are to clarify information provided in the proposals Will include a Question and Answer period at the end of the Presentation Invitations are at the sole discretion of the University

10 Scope of Work Brief Summary of Project Requirements and Specifications: Audio Visual Equipment and Installation/Integration for a conference room and Classroom for the Perinatal Research Branch, Hutzel Women’s Hospital: The current ceiling mounted projector in the PRB classroom cannot accommodate today’s high definition digital images that are created within the PRB. These images must be downgraded, the resolution lowered, to enable them to be shown from the current projector. The installation of two flat screens high definition monitors into the PRB classroom will improve the display of digital images shown during daily clinical rounds and scientific presentations. These graphics include 2D, 3d and 4d ultrasound studies, MRI images, macro and microscopic pathology specimens, digital photographs and digital video case presentations as well as presentations delivered by local, national and international speakers. During daily clinical rounds and presentations questions are raised about images, procedures etc. All participants, fellows and staff, bring their personal laptops to search for answers via Pub Med, Google Scholar and other sources of information. Often there are multiple papers, video and web sites that provide answers and input into the discussion. The ability to display and switch between multiple sources will increase flexibility and stream line the process of sharing this research data.

Additional: Voluntary Alternate is acceptable; however base bid must be responded to first and additional Cost Schedules must be labeled as “Voluntary Alternate” to be considered. A loading dock is available, as well as, freight elevator to the 4th Floor multi-usage room Freight elevator should be padded to avoid damage. A dumpster is not anticipated, vendor to remove all refuse and packing materials at end of work day. Recording or storing of recordings is not required; however, recorder will be used for streaming. Streaming up to approximately 5 participants with watch/listen capability only (no feedback). No new network drops required. Laptop “plus in” to be located on the computer cart. Projector is to be replaced in the ceiling. Owner to provide replacement ceiling tile to cover the cavity. No dedicated document camera. Note thumb drives will be used. Inputs from laptops for presentations are to feed both screens

Vendor must have in-house AMX Programmers. There is not signal flow diagram available at this time. No current AMX Code available. Pre-existing computer will stay. A microphone is to be added to the podium and all microphones are to be able to function at the same time. Removal of white board, in wood case, suspended from front wall to be removed by owner and patched by same. The projector screen will merely be moved up into the ceiling well. 2 monitors will be mounted to the front wall, side – by – side. This is an occupied building and all precautions for noise, dust, vibration, disruption to foot traffic must be employed. Outputs are DVI and VGA on computer

1. Compliance with WSU Standards for Communications Infrastructure  A. All applicable work, products, materials and methods shall comply with the latest version of the “WSU Standards for Communications Infrastructure” except as where noted. B. This document is available at the following website/URL: 2. Automation System Program Code A. All automation system uncompiled and compiled program codes, source codes, custom modules, graphical user interface screen shots and any other automation system programming data and material (Program Code) shall be provided to the UNIVERSITY in hard copy and on CD Rom in an unencrypted format acceptable to the UNIVERSITY. B. Copyright for the Program Code shall be assigned to the UNIVERSITY for purposes of system maintenance. C. Provision of and Copyright assignment of the Program Code to the UNIVERSITY by the Vendor shall be conditions of the Purchase Order and contract acceptance by the Vendor. D. Provision of and Copyright assignment of the Program Code to the UNIVERSITY by the Vendor shall be conditions of final System acceptance by the UNIVERSITY

14 Proposals Must Include
Organizational data that demonstrates the size, scope and capability of the Company A complete description of the products and services offered Vendor’s qualifications to meet the RFP specifications

15 Information Submitted on Request
Publicly distributed annual reports and/or independently audited financial statements A minimum of three (3) qualified references Lost Accounts with annual billings of $25,000 per year

16 Contracting Process Any contract will be made using the University’s Standard Service Provider Agreement Agreement will incorporate this RFP and its terms and conditions by reference Any Vendor terms to incorporate into the Agreement must be included in Exhibit 1 Final contract will require approval by the University’s General Counsel

17 Other Information All Proposals are subject to the State of Michigan Freedom of Information Act If awarded a contract, VENDOR must provide a Certificate of Insurance as outlined in Schedule B Vendors should include any Supplier Diversity information (self or sub-contractors) Construction or construction like work must be performed at Prevailing Wage Rates Wayne State has a smoke- and tobacco-free policy for indoor and outdoor spaces
