1 How to use your Sewanee Student Patient Portal

2 You can find your Patient Portal at: https://wellnessweb.sewanee.edu This is your Patient Portal home screen. The log-in should prompt you to enter your username and password. Your username is your Banner Username, or the first part of your email address. For example, if your Sewanee address is patientte0@sewanee.edu...

3 You can find your Patient Portal at: https://wellnessweb.sewanee.edu patientte0 Your password is the same password you use to access Banner. Then hit Proceed. ****** Having trouble? Make sure you didn’t put www in your address. Make sure you used your Banner password, not the password for your Sewanee e-mail.

4 The Portal will then prompt you to confirm your identity by putting in your Banner ID Number. Your Banner ID Number is your student number that starts with B 0 0. For example, if your Banner ID Number is B00123456 … B00123456 Then hit Proceed. Make sure you include the B00 portion of your number.

5 This is your Patient Portal Home Screen. From here, you’ll be able to … Schedule certain appointments on your own at anytime Manage those appointments Send Secure Messages to providers Get messages from the Wellness Center, like results from labs you’ve had done at the Health Service Fill out required paperwork for Wellness Center appointments online and before your appointment time

6 The tabs on the side will guide you to different functions of the portal:

7 Appointments What can I do on the Appointments tab? Some types of visits can be scheduled automatically with Health Service (UHS). » Allergy Shots » STI Testing » Doctor’s Visits for Injuries – like an ankle or wrist injury » Doctor’s Visits for some Illnesses – like having a sore throat » Appointments to get Emergency Contraception » and more! Some types of appointments can’t be scheduled automatically, but you can initiate contact with a staff member to set up an appointment time. » Counseling and Psychological Service (CAPS) therapy appointments – Replaces emailing CAPS since e-mail is not a secure means of communication Cancel UHS appointments ( > 24 hours before your appointment) View appointment information – when your appointment time is scheduled, with whom Fill out required paperwork online from the comfort of your dorm room!

9 Scheduling Appointments Choose the type of appointment you need and follow the prompts.

10 Scheduling Appointments After answering the required prompts, you’ll be able to select an available appointment time.

11 Scheduling Appointments After you select an appointment time and click “ Continue, ” you’ll come to a confirmation screen. The appointment is not recorded before this screen, so you MUST click “ Book ” to finalize and secure your appointment time.

12 Scheduling Appointments After scheduling the appointment, you’ll be guided to a questionnaire – you can fill this out immediately or at a later time (before your appointment). You must click “ Submit Final ” in order to submit your questionnaire. ✓ 8/23/16 ● ● ● I was walking back to my dorm Tuesday night and stepped off a curb wrong. My ankle was swollen that night. It’s not now but it hurts when I put my weight on it.

13 Scheduling Appointments After you complete the questionnaire, review it and click “ Proceed, ” you’ll be able to see a summary of your appointment. You can cancel the appointment here if it’s before 24 hours prior to the appointment time.

14 If you schedule an appointment for UHS, CAPS or DSS – online or on the phone – check your portal! You may have a questionnaire that must be filled out prior to your appointment.

16 Secure Messages What can I do on the Messages tab? Contact the Wellness Center and some of its staff. o E-mail is not a secure method of communication. The Secure Message function of your Patient Portal solves this problem! o Ask the Insurance Coordinator a question. o Contact CAPS about appointments instead of calling. o Request a copy of your current Accommodations Letter. o Etc Receive messages from the Wellness Center. o Lab results can be sent to your portal as soon as they come in. You’ll receive an automated notice in your e-mail to alert you to a new message on your Patient Portal.

17 Secure Messages Any messages you’ll get from Wellness Center providers will appear in your Inbox. Click “ Read It ” to read the message and reply to it. A check mark will appear once the message has been read. To send a new message, click the “ New Message ” button at the top.

18 Secure Messages Choose the type of message you need to send and press “ Continue. ” There are some limitations for each type of message; please be sure to read any instructions before sending a message. This will prevent any delays in your message getting to the proper staff member.

19 Secure Messages Fill in the prompts and hit “ Send ” to send your message. If there’s a required element, the Portal will ask you to fill out that section before proceeding. All required parts are marked with a **. Question about my coverage Ms. Bradford, my parents have changed their insurance – should I send you a new copy? Does it affect my services at the Wellness Center? 931-598-0000 ✓

20 Wellness Center staff members can only check Secure Messages during open business hours. (Monday – Friday 8 – 4:30 p.m.) Please allow at least 1 business day for a reply. Do NOT send a secure message in an emergency! Call 911 instead!

21 Other What else can I do? See and manage Wellness Center groups and workshops you sign up for. Take seasonal surveys, when available, to give feedback on our services. And More! The Patient Portal may have additional features added in the future. Any updates will be announced on the Home page.

23 Remember: If you’re accessing your Portal in a public location, LOGOUT before leaving the computer to keep your information private and confidential. Do not give your Banner ID information and password out to anyone. If someone knows your password and your Banner ID Number, they can view your Portal. If you’re not able to schedule the type of appointment you want or need assistance, you can always call the Wellness Center at 931-598-1270.
