Use the risk factor and intervention in Worksheet 4.4: Case Study – Implementation.  Determine a short-term outcome for that risk factor.  Write your risk factor, intervention, and short-term outcome on three large post-it notes (one color per risk factor).  Each group will post their risk factor, intervention, and short-term outcomes in the appropriate column of the logic model in the front of the room. 28
