HSE Safety Alert – Potential Injury (Incident No: ) DISCUSSION On the 8 th January, 2015 an ASPEC Sub-Consultant reported that whilst conducting a site visit, with a Client’s Site Supervisor, they were requested to provide JSEA to accompany their Take 5. The Site Supervisor had previously advised our Sub-Consultant that the Take 5 was sufficient. The Site Inspector advised that a JSEA was required to accompany the Take 5. The site visit was terminated immediately. No harm was caused to the ASPEC Sub-Consultant, other team members or Client Employees. Actual Consequences of incident: Breach of Procedure, liability exposure. ACTION The incident was discussed with immediate Supervisor and ASPEC Management. The following should be undertaken as a minimum for ALL ASPEC Engineering staff: Reminder to complete an ASPEC JSA along with an ASPEC Take 5 for appropriate Site Inspections. Do not reply upon the Client follow their systems. Reminder to being prepared is the right step toward being safe, completing the task at hand and ensuring you have adhered to ASPEC requirements, as well as our Clients. All ASPEC staff have a legal obligation to ensure they are familiar with the components of the HSE Management System, abiding by ASPEC HSE procedures, reporting and risk managing hazards and reporting incidents/ near misses

HSE Activities Client Safety Audit Newcastle Office is under going preparation for an annual safety audit by Newcastle Coal Infrastructure (NCIG) in July. Areas of improvement are Environmental Management, ASPEC Inductions and as precaution, ensuring that all documents are current and revised. Site Clothing Should anyone require further supplies just send me an . A REMINDER to check your Hard Hat. Is it still in date? First Aid Kits Don’t forget if you have accessed the first aid kit, take the time to think - ‘Am I using this due to a near miss or incident?’. If so we need to be reporting them. HSE Activities Monthly Office Inspections Has your office completed it’s month SAFETY Inspection? No? How about we do one now? Annual Fire Drill Has your office completed it’s annual fire drill? Don’t forget to up-date the details in the ASPEC Fire Drill Register First Aid First Aid Certificate for First Aid Officers have now all been completed in Brisbane & Newcastle. Fire Extinguisher Inspections Has your office completed this recently? Would it be more than 6 months ago? Must be completed every 6months and the ASPEC Inspection Register updated. Quality Activities The Quality System Recertification Audit was held in Brisbane on 6 th January. All actions required to be followed up by 30 th January have been submitted. Awaiting written confirmation of re-certification. Preventative and Corrective Action Can you think of any areas of improvement that you would like to raise?
