3. ASPEC QSHR August 2014 Minutes INTEGRAL July 2014 DISCUSSION On the 7 th July, 2014 an INTEGRAL employee reported that whilst driving he hit a clearance bar in a carpark entry. The employee stopped and assed the damage immediately. Due to the impact the plastic cover of the beacon was smashed. The driver is expereinced. The drives advised that due to new tyres, it had increased the height of the vehicle. Nobody was reported as injured. Actual Consequences of incident: minor repair Corefleet. Potential Consequences: more severe damage to vehicle and possibly driver. ACTION The incident was investigated by INTEGRAL Management Reminder to always visually inspect vehicle and complete pre-start on a weekly basis Review INTEGRAL Engineering HSE Management System Procedure 7.3 Vehicles and Driving All INTEGRAL staff have a legal obligation to ensure they are familiar with the components of the HSE Management System, abiding by INTEGRAL HSE procedures, reporting and risk managing hazards and reporting incidents/ near misses
